Parents & Guardians! Be sure to submit your student's shirt sizes on this form.
You may need to size up for a good fit for their FREE RCDS shirt they will wear on field trips and to represent their school!
Reminder: Current Immunization records are required to attend school. Please send immunization records to [email protected] or bring them by the front desk of the main building. Contact Amy Alley if you have questions.
